    • You must be age 65 or older, unless you have a disability or permanent kidney failure
  • * Meet “homebound” status, which means:
    • Your condition should be such that there is a normal inability to leave home and consequently, leaving home would require a taxing effort
    • Absences from the home are infrequent, of short duration or are for the purpose of receiving medical care
    • You do not have to be bedridden to be considered confined to your home
  • Be under the care of a physician who establishes the plan of care
  • Need at least one of the following skilled services as certified by a physician:
    • Intermittent skilled nursing services that meet the criteria for skilled services
    • Physical therapy services
    • Speech language pathology services
    • Occupational therapy services
  • Home health serves much be furnished by a participating Home Health agency

Cost and Payment

Payment for services can be provided through Medicare, Medicaid, Workman’s Compensation, Veterans Administration Insurance, private insurance or private pay. For most services, there is no cost to the patient if eligible for Medicare Part A or Medicaid. Insurers may require pre-certification and may limit the number and type of home health visits that we can provide.

Any charges for services not covered will be discussed with you prior to giving these services. The patient, guardian, caregiver or family member will be informed of all charges for services provided and methods of payment.
